Juan Carlos Villamizar
CEO & CO-FOUNDER
Juan Carlos Villamizar is the CEO and Co-Founder of CHDFS, Inc. * Through his career, Juan Carlos has dedicated his work towards advocating for the needs of children in various capacities. He has served government roles as a Developmental Sociologist, Sociology Consultant, and also has field experience in Family Support and Teaching capacities. Juan Carlos holds a Masters of Social Work/SEA degree from Columbia University, a degree in Psychoanalysis, and a Bachelor of Science in Sociology from the University of Antioquia, Colombia.

Sobeida Valdez
CHIEF OPERATING OFFICER
Sobeida Valdez joined CHDFS, Inc. in 2014 in the role of Human Resources Director bringing with her over twenty years of experience in social work, case management, advocacy and other human service roles. She brings a mix of managerial, financial and operational experience which has contributed to CHDFS’ growth. In her current role as Chief Operating Officer, Sobeida oversees CHDFS’ service programs providing guidance, support and strategy to ensure best practices and quality service delivery.
Prior to joining CHDFS, Sobeida held various roles of increasing responsibility in both non-profit and proprietary agencies. Sobeida is a Licensed Master Social Worker and a graduate of Hunter College School of Social Work. In addition, she holds a Master of Science degree in Human Resources Management with a concentration in Organizational Development from the The New School for Social Research and Bachelor of Arts in Psychology from City College of the City University of New York.
Rosa Caccamo
CHIEF FINANCIAL OFFICER
As the CFO, Rosa oversees the organization's financial transactions, prepares current financial reports and summaries, and creates forecasts predicting future growth.
Rosa joined CHDFS Inc. as a Finance Assistant in February 2011. Before joining CHDFS, Rosa worked at Ernst & Young as an Audit Senior for five years. Her experience includes performing external audits for Mining and telecommunications companies. Rosa is an accountant who holds a B.S. in Accounting.
Jhon Cortes
HR MANAGER
Jhon Cortes has many years of experience working in the hospitality industry. In 2014, he joined CHDFS and brought with him substantial knowledge of management, coordination, scheduling, organization, and problem-solving. Since joining CHDFS, he has held positions in recruitment, training, and intakes/placement. Currently, Jhon is in the role of HR Manager helping to support and implement human resource policies and programs that will contribute significantly to CHDFS's continued growth. Jhon is passionate about nature and enjoys traveling. He wishes for a sustainable relationship between humans and our planet.
David Tavarez
QA/QI DIRECTOR
David Tavarez joined CHDFS Inc. in 2017 in the role of Outreach and QA/QI Coordinator for the Health Home Program. In January of 2019, he was promoted to Quality Assurance and Quality Improvement Director. David has a background in working with underrepresented minority college students in the areas of outreach, mentorship, and academic advisement while working at the Urban Male Leadership Program at Lehman College through the CUNY Black Male Initiative.
Mr. Tavarez is a Lehman College graduate, earning his Bachelor’s Degree in Business Administration with a concentration in International Business & Human Resource Management in May 2014. David has received various awards, including The CUNY Vice Chancellor’s Excellence in Leadership Award for 2013-2014, Champion of Service Award for 2013-2014, Outstanding Leadership in Service by a Graduating Senior for 2013-2014.
Leslie Cortes
SERVICE COORDINATOR SUPERVISOR
Leslie Cortes joined CHDFS, Inc. in 2015 as an OMH waiver service provider working with children. In 2017, she joined the Early Intervention team as a service coordinator. She also has experience working with dual EI/Health Home cases. In 2020, Leslie was promoted as SC supervisor.
Leslie holds a Bachelor's of Business Administration in I/O psychology and is working towards her Masters in Social Work at Silberman School of Social Work at Hunter College
Glennis Rodriguez
HEALTH HOME DIRECTOR
Glennis joined CHDFS, Inc. in 2019 as a Waiver Service Provider. She was subsequently promoted to Health Home Care Manager in 2021. Glennis has been instrumental in the growth of our Health Home program and was recently promoted to Health Home Coordinator overseeing her own team. Glennis has a background in working with children in foster care. She is passionate about providing guidance and assistance to children and families in order to help them succeed. Glennis has a Bachelor in Social Work from Concordia College and has a Master in Social Work from Yeshiva University.
Edison Gaston
HEALTH HOME ASSISTANT DIRECTOR
Edison joined CHDFS Inc. in 2019 as an Intake Coordinator after graduating from Lehman College with a Bachelor’s degree in Sociology and Psychology. Within the same year, he was promoted to Health Home Care Manager for the following 4 years. Edison is now the Health Home Program Assistant Director where he supervises a strong, dedicated and caring team of Care Managers to provide families with the resources needed to enhance their medical and mental health. He is currently pursuing his Master's Degree in Social Work at Lehman College.
Valerie Tain
HOMEMAKING PROGRAM DIRECTOR
Valerie joined CHDFS in 2018. She brings with her 23 years of managerial experience in homemaking services from her former job. She attended John Jay College of Criminal Justice, CUNY, and graduated with a BA, specializing in Deviant Behavior and Social Control. She also has a Masters in Public Administration (MPA) specializing in Productivity. She began her career at CHDFS as a Health Home Care Manager working with Seriously Mentally Ill (SMI) Adults. She was later promoted in January 2021 as the Director of the CHDFS Homemaking. In the homemaking program, she oversees all the daily operations of the program which works with referred families from NYC Administration for Children Services.
Asher Weis
HR & RECRUITMENT SPECIALIST
Asher Weis is a transplant from Colorado. Although he's lived in NYC long enough to be considered a New Yorker. His experience is diverse, from teaching in elementary school, managing restaurants and big box retail. Now Mr. Weis is our HR & Recruitment Specialist. He works with recruiting, on-boarding, training and anywhere else he is needed.
Nancy Parker
PARENT ADVOCATE DIRECTOR
Since 2013, Nancy Parker has served as the Director of the Parent Advocate Program at CHDFS, Inc. Her relationship with our organization goes back nearly thirteen years where she's fulfilled numerous supporting roles including serving on the Board of Directors. In the past, Nancy has worked as a Parent Advocate at NAMI NYC Metro, where she began and ran a support group for fourteen years, targeting parents of children and adolescents; dealing with the challenges of having kids suffer from emotional and mental disorders.
Nancy is a state trainer of NAMI’s signature course, NAMI BASICS, and is a Family to Family teacher. Nancy also runs a volunteer social group for adults with mental illness called "Find Your Funny." Nancy has a Bachelor's Degree from Hunter College.
WAIVER PROGRAM DIRECTOR
Vanessa Sosa joined CHDFS Inc. as a Care Manager in August of 2018. She holds a Bachelor of Arts from the College of New Rochelle and is currently working on her Master in Social Work at Yeshiva University. She has over 23 years of experience in working with OPWDD advocating for people with developmental disabilities. While at CHDFS, Vanessa was promoted to the position of Health Homes Assistant Director in January of 2019. Most recently, she was promoted to the Director of Waiver Services in January 2024.
